A Chronic Disease is a condition that has, or is likely to last for 6 months or more.
Conditions include, but are not limited to: Cancer, Diabetes, Asthma, Arthritis, Cardiovascular illness, Mental Illness, Stroke & Musculoskeletal conditions.
At Caboolture Community Medical, our aim is to prevent our patients succumbing to the deteriorating effects of chronic disease by providing them with a GP Management Plan.
This plan ensures our patients receive the highest level of ongoing care and may enable access to Allied Health Services providers such as physiotherapists, diabetes educators, dieticians, dentists, psychologists, Audiologists, chiropractors and Podiatrists.
